LIFE Premium Points Program

The LIFE Premium Points Program is an annual incentive program for all associates and spouses enrolled in the J. J. Keller health plan as of January 1 each year. 

The company’s philosophy over the years has been “shared responsibility and shared results.” By participating in the LIFE Premium Points Program and Annual Health Assessment, health plan members can earn an additional company premium contribution toward their health plan premiums. Without participation in the LIFE Premium Points Program, an associate or spouse can expect to pay the Base premium rate.

LIFE Premium Points Program Year

The LIFE Premium Points Program year runs from October 1-September 30 annually. All activities and points must be earned and recorded by September 30 every year.


October 1 – September 30

All points must be recorded and entered no later than September 30. At this time, points are calculated and health premium levels for the follwing year are determined. Associates are notified of their earned premium level in their open enrollment letter sent in mid-October. While the new program year starts on October 1, there is a blackout period in the Marathon Health Incentive Portal when points cannot be recorded.

Annual Health Assessment Requirements

Each year, a completed Annual Health Assessment is required to be eligible for additional money from the company. This includes a biometric screening and online Health Assessment Questionnaire by August 31.

Earning Points and Tracking Incentives

There are several ways to earn LIFE Premium Points throughout the program year. However, to be eligible for any level of additional company funding, you must complete an annual Health Assessment and Health Assessment Questionnaire online. 

Points are tracked in your Marathon Health Portal. Some points are self-recorded while others are recorded automatically.

  • Health Assessment Questionaire
  • Health Assessment Biometric Values
  • Appointments and flu shots provided by a Marathon Health Provider
  • Chronic Condition Management Program completion
  • Health Coaching with a Marathon Health Provider
  • Tobacco Cessation Program completion
  • Marathon Health Workshops
  • Fitness Minutes recorded by a Connected Device
Self-Recorded Activities
  • Annual Physicals, Cancer Screenings and Vaccines NOT performed by a Marathon Health Provider
  • Blood Donations
  • Dental Check-Ups and Vision Exams
  • Marathon Health Webinars
  • Double Heart Provider Form
  • Fitness Minutes NOT recorded by a Connected Device
  • Wellness Challenges

Lower Health Plan Premiums

Depending on the health plan you choose and the LIFE Premium Point level you achieve each year, the additional company funding you receive will vary.

The level that you were initially gifted when new to the health plan is equivalent to completing both steps of the Health Assessment process AND earning 120 points within the program year (Best – Level 3).

  • Base = $0 additional company funding
  • Level 1 (Good) = $600 total
  • Level 2 (Better) = Additional $600 ($1,200 total)
  • Level 3 (Best) = Additional $800 ($2,000 total)

Please know that if you do NOT participate in the LIFE Premium Points Program, you will lose the additional money from the company that is currently lowering your premium and your premiums will automatically increase for the upcoming plan year.


Alternative Standard Request

The J. J. Keller wellness program is committed to helping you achieve your best health. Rewards for participating in a wellness program are available to all employees. If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity to earn the same reward by different means. Contact the Health and Wellness Center at ext. 7599, and the staff will work with you (and, if you wish, with your doctor) to find a wellness program with the same reward that is right for you in light of your health status.  Alternative standard requests must be made to the LIFE Wellness Center by March 31, 2023.
